## 1. Review Title and Registration

## 3. Eligibility / Search / Selection

Inclusion: phase III pivotal, adults with active SLE (seropositive) or lupus nephritis, belimumab vs matched placebo, composite response primary endpoint (SRI-4 or PERR). Exclusion: post-hoc EMBRACE (African-ancestry SLE sub-cohort, smaller); phase II dose-finding (LBSL02); paediatric PLUTO. Two-reviewer adjudication; PRISMA 2020.

## 6-7. Data Extraction + RoB

Per-arm response counts + RR (log-RR scale), steroid-sparing effect (>=25% reduction from baseline), time-to-severe-flare (hazard ratio), complete renal response (BLISS-LN), serious infections, depression/suicidality pooled signal. RoB 2 LOW across D1-D5 for all 4 trials (central IVR, double-blind matched placebo, central SLEDAI/SELENA training, central lab for renal, SAP locked). Author sign-off per provisional-RoB banner.

## 9-10. GRADE / Reporting

Standard GRADE. SRI-4 composite at 52 weeks (BLISS-52/76/SC) and PERR at 104 weeks (BLISS-LN) are distinct but validated SLE response measures — pre-specified GRADE indirectness downgrade for endpoint heterogeneity. Effect highly consistent across trials (RR 1.27-1.34). Disease-stage heterogeneity (non-nephritis SLE vs active lupus nephritis) is a clinical feature, not a flaw. Pooled effect (RR 1.29, 1.17-1.42) approximates the class-level treatment effect when added to standard of care. PRISMA 2020, CONSORT-Harms.
