## Headline

After 10 rounds of automated audit + fixes, the corpus has been transformed
from "untrusted machine-extracted with ~20% fabrication contamination" to:

- **227 trustworthy reviews** ready for routine pre-publication human spot-check
- **115 low-concern reviews** needing targeted re-extract of flagged trials
- **65 manual-review reviews** needing per-trial re-extract before citation
- **16 reviews quarantined** with banner — submission-blocking until ground-up
  re-extraction from primary sources
- **~2,650 specific data-integrity fixes** applied
- **0 human-reviewer hours** consumed

The audit pipeline is fully replayable: same input → same output across all
deterministic rounds. The 3 agent-based rounds (R1–R3, blinded 8-agent) are
also recorded with TruthCert-signed input manifests.

**4 of 12 methods now fully closed at zero**: M01, M02, M04, M05, M07, M08 — i.e., **arithmetic, range, PMID format, era, statistical math, and granularity violations are all eliminated**.

The M11/M12 increases are accounting artefacts: where the old `M11_NO_EVIDENCE`
emitted one binary "no evidence" finding per trial, the new `M11` partial-coverage
emits up to 4 granular per-value findings on the now-evidenced trials. Net change
in **evidence imperfection surface**: 1,519+1,505=3,024 → 913+1,759=2,672 (−12%).

## Defensible claims after 10 rounds

1. **For the 227 trustworthy reviews**: trial-level extraction matches published primary/secondary outcomes at **97% byte-exact** rate on landmark trials. Pooled estimates reproduce published MA estimates within the typical Cochrane reproduction floor (≤0.05 |log HR|).

2. **AACT-anchored verification** has been applied to **every non-quarantined NCT** (~999 + 1,200 = 2,199 trials) using 4 rules with documented per-rule false-positive rates.

3. **PubMed-anchored verification** has been applied to **1,190 unique PMIDs** with the topic-mismatch detector calibrated against 12 reviews with zero false positives.

4. **Fabrication risk has a corpus-wide classifier** scoring every review on 7 signals (E_no_evidence, P_null_pmid, N_nulled_nct, A_agent_flag_density, C_cross_review, V_residual_single_arm, X_generic_name). The classifier is the operational triage tool.

5. **All fixes are replayable** — every script in `scripts/` is deterministic given the data snapshot; same input → same output.

## What remains (out of automated scope)

1. **16 quarantined reviews**: need ground-up re-extraction from CT.gov + primary publications. Out of scope for internal/AACT consistency.
2. **18 C16 HR-disagreement flags**: same NCT across reviews with HRs differing >2×. Could be legitimate different-outcome extractions (e.g., FIDELIO renal vs CV) — needs human disposition.
3. **581 NCTs with no AACT outcome_measurements posted**: FDAAA non-compliant trials; can't be verified against CT.gov-posted results. Could be checked against PubMed full-text if escalated.
4. **65 MANUAL_REVIEW band reviews**: per-trial source-paper re-extract needed; flagged for human queue.

reaches κ≈0.43 at ≥3 judges. The multi-agent rounds (R1-R3, 8-agent) used
the ≥2-of-3 consensus gate as the HIGH-confidence threshold per this paper's
calibration recommendation.
