In adults with acute or recent venous thromboembolism, what is the effect of apixaban compared with conventional anticoagulation, another direct oral anticoagulant, or placebo on recurrent venous thromboembolism and on bleeding?
In this section: Registration and administrative information (PROSPERO field set)
| Review title | Apixaban for the treatment of venous thromboembolism: eight trials report an outcome named for recurrent VTE, and three of them count the same thing |
|---|---|
| Review question (PICO) | In adults with acute or recent venous thromboembolism, what is the effect of apixaban compared with conventional anticoagulation, another direct oral anticoagulant, or placebo on recurrent venous thromboembolism and on bleeding? |
| Population | Not shown — doac_vte_nma is a network protocol whose intervention is the DOAC network, not apixaban VTE treatment alone. State: PROTOCOL_IS_A_NETWORK. |
| Intervention | Not shown — doac_vte_nma is a network protocol whose intervention is the DOAC network, not apixaban VTE treatment alone. State: PROTOCOL_IS_A_NETWORK. |
| Comparator | Not shown — doac_vte_nma is a network protocol whose intervention is the DOAC network, not apixaban VTE treatment alone. State: PROTOCOL_IS_A_NETWORK. |
| Background / rationale | Not recorded — this object holds no background field, and an introduction generated without one would be argument that no source in this review supports |
| Eligibility criteria | Population: adults with acute or recent venous thromboembolism being treated for it. Intervention: apixaban as the randomised intervention -- the drug itself, not a class of which it is one member. Comparator: conventional anticoagulation, another direct oral anticoagulant, or placebo. Estimand, which governs poolability and not eligibility: recurrent venous thromboembolism, or major bleeding, at any registered rank. And the boundary with the prophylaxis review: extended anticoagulation in patients who have already had a venous thromboembolism is treatment and belongs here; primary prophylaxis in patients who have not is prevention and belongs to the sibling. The discriminator is read from each trial's eligibility criteria, because the registry's coded `primaryPurpose` does not carry it and both conventions are used for both situations. Criteria are derived post hoc and say so. |

This assessment is DUAL — two assessors from different model families, the second asked blind. The agreement rate is withheld pending adjudication. Both readers were given this review’s decision rule; an earlier version of this sentence said the blinding had withheld it, and that was wrong. What the 15 domain comparison(s) show is a reading difference that runs in OPPOSITE directions by domain — on domains 1 to 3 the second assessor more often declines to judge at all, and on domains 4 and 5 it more often judges low. A single agreement rate averages those two behaviours into one number that describes neither. Both assessors’ per-result judgements are shown in the table above, unadjudicated, and neither is this review’s finding. No adjudication has been performed, so this review holds no final risk-of-bias judgement for these results.

Favours: neither -- the interval spans the null
computed; tau-squared is 0 and at k=3 that is an estimate from almost no information, which is why the interval is Knapp-Hartung on 2 degrees of freedom rather than a normal one
Three trials randomise apixaban itself against another anticoagulant for the acute event and post an arm-level count of recurrent symptomatic VTE with no death term. Their definitions were read and compared, not their names.
Read this beside the estimate. Not one of the three contributing trials masks anybody: NCT03045406, NCT03266783 and NCT01780987 all register masking: none. The endpoint is adjudicated on all three, which is the standard mitigation for an open-label anticoagulation trial, but no registration states that the adjudicators were blinded.
On NCT03266783 the registered primary is clinically relevant bleeding and recurrent VTE is the third of nine secondaries. On NCT01780987 the registered primary is major bleeding and recurrent VTE is the first of five. Only CARAVAGGIO contributes its own registered primary.
Dalteparin, rivaroxaban, and unfractionated heparin followed by warfarin. The pooled quantity is therefore apixaban against whatever else was given, which is one step from any question a reader has. This object's GRADE rates indirectness down for it and the certainty here is very LOW.

The death term is part of the definition and not a detail. Five of the eleven trials register a primary composite carrying one, and they do not carry the same one: Amplify counts VTE-related death, AMPLIFY-EXT counts ALL-CAUSE death. Same sponsor, same programme, sequential trials, names differing by a hyphenated suffix.
What each trial COUNTED, in its own registry record's words, before anything here was pooled. A sentence saying what HAPPENED is not a sentence saying what was COUNTED, and only the second one licenses a pool. Read from the registry on 2026-08-19. Does the reported estimate depend on any one of the three trials? At k=3 that is not a formality: the largest contributor is more than seven times the size of the other two together.
| Cohort omitted | k | RR (95% CI) |
|---|---|---|
| CARAVAGGIO | 2 | 1.021 (0.0105 to 99.38) |
| COBRRA | 2 | 0.6904 (0.0419 to 11.37) |
| Japanese acute DVT/PE study | 2 | 0.7859 (0.0681 to 9.069) |
AT k=3 the pool is largely one trial. CARAVAGGIO contributes 78 of the 108 events in this pool; its two companions contribute 30 between them. Omitting it moves the point estimate from 0.776 to 1.021, and the span across all three leave-one-out fits is 0.690 to 1.021. All three intervals span the null, so no single trial is holding the conclusion up -- and none is holding it down either. The honest reading is that this estimate is a description of CARAVAGGIO with two small trials attached.
Cochrane Handbook 10.14: sensitivity analyses ask whether the conclusions are robust to decisions made during the review. Here the decision under test is the inclusion of each individual trial, not the estimator -- the estimator comparison is reported separately at `estimator sensitivity`, because they are different questions and presenting them in one table would let a stable point estimate under one read as stability under the other.

Submission formats written to the figures directory, not embedded here because an uncompressed 600 dpi TIFF is tens of megabytes: EPS (vector, journal's first choice for line art) — forest_fit.eps, 4 KB; TIFF (uncompressed, 600 dpi -- journal fallback) — forest_fit.tiff, 14,370 KB.
Changing the range moves the axis window only. The guides stay labelled with the null and the extremes of the plotted intervals, so no plotted value and no printed number differs between these views — and that is checked at build time, not asserted. Ranges not offered because they would crop the data: 0.5 to 2, 0.25 to 4, 0.7 to 1.3.

Projected from the canonical object, so it carries the same k, the same pooled estimate and the same interval as the paper and cannot drift from them. The interval is drawn CROSSING the no-difference line because it does: 0.344 to 1.753 spans 1. A graphical abstract travels without its caption, and one showing a favourable point estimate without showing that its interval includes no effect would overstate a null result.
⬇ SVG (vector) 13 KB
Only the vector format is offered for this figure, by choice: it is line art — discrete marks, glyphs and labels — which SVG reproduces exactly at any size, so a raster would add weight without adding fidelity. The build was able to rasterise; it was asked not to.
Every cell carries BOTH independent cross-family assessments and they are not reconciled: showing one column would be a reconciliation presented as an observation. Glyph as well as colour, so the panel survives greyscale printing and colour-blind reading. This is derived from what the object holds now.'}.

Deliberately not drawn. Every subset meta-analysis of 3 trials is 7 points; a cloud that small cannot show the multimodality GOSH exists to reveal, and a reader would take the picture as evidence of its absence.

TSA boundaries depend entirely on a target information size that must be pre-specified. This review's protocol registers none, so any boundary drawn here would be a parameter chosen after seeing the data -- which is the practice TSA exists to protect against.

The protocol pre-specifies no moderator, and at k = 3 a meta-regression would be fitted on 3 points. Not drawn rather than drawn with a caveat: a bubble plot invites reading a slope, and there is no slope here that any reader should read.

Statements this review records about the limits of what it establishes, which are not shown elsewhere on this page. They are reproduced from the object verbatim and are not summarised.
| Why empty is a result and not a blank | The boundary criterion assigns every trial to exactly one of the two reviews, so the intersection is empty by construction and the emptiness is computed rather than left as a bare `[]`. P17. The corpus-level consequence is the good one: summing k across these two topics does not double-count, which is the opposite of the ablation split's situation. |
|---|---|
| What asking it bought and what it did not | It bought the review its entire pool. All three contributing results were found by asking at every rank: two of the three are secondary outcomes in trials whose registered primary is a bleeding endpoint, so a review reading primaries only would have reported that they contribute nothing. It did not buy poolability: eight trials answered yes and five of the eight still do not pool; the coherence screen records why: NCT00643201 and NCT00633893 have no recurrent-VTE measure without a death term, NCT04168203 and NCT03196349 are extended-anticoagulation trials rather than acute-treatment trials, and NCT02744092 posts a percentage for a drug-class contrast rather than an arm-level apixaban count. The prophylaxis half of this pair ran the identical discipline and its poolable set went 3 -> 8. Here it went 8 -> 3. The method has no direction -- asking is not a way of finding more trials, it is a way of finding out. |
| What it does not establish | Not that either pool is correct -- both were judged by reading endpoint definitions, and that judgement stands or falls on its own. Not that the direction is unpredictable in general; two instances are two instances. What it establishes is narrow and it is the thing that was missing: The procedure has no built-in direction, demonstrated rather than asserted. |
| Interval method superseded | Paule-Mandel with a floored Knapp-Hartung interval on k-1 df, the same as the pool itself. At k=2 that is t on one degree of freedom, whose critical value is 12.7 -- which is why these intervals are enormous and why they should be. |
| Refused because | Three of the eight register a bleeding endpoint as their primary -- COBRRA, covet and the Japanese study. Pooling 'each trial's own primary' would average a harm against a benefit, and the pooled quantity would have no direction: a risk ratio below one would mean fewer thromboses in five trials and fewer bleeds in three. This is the only declined pool in this file that is refused as incomputable rather than computed and set aside, and the reason is stated so a reader can check that it is a reason and not a preference. |
| What was refused | reconstructing a recurrent-VTE-alone estimand for amplify and AMPLIFY-EXT by summing the components they post separately |
| Why refused | The components sum to 61 and 76 against a posted composite of 59 and 71. A composite counts a participant's first event; its components count participants. Summing them would have manufactured 2 and 5 events that the trial's own adjudication does not record, and the manufactured events fall in the comparator arm at more than twice the rate they fall in the apixaban arm -- so the derivation is not merely imprecise, it is biased in the direction that flatters the drug. |
| What it does not establish | Nothing about whether either pool is coherent. P36 stands unchanged: the coherence judgements in this file were made by reading definitions and would be identical if every I2 came out zero. |
| Why absent | k=3 and a pool was computed -- it is on this page -- but not by a model call this object can quote. There is no R session, no printed output and no package version, so P6 refuses on the quotation and not on the synthesis. The arithmetic is reproducible from the script named on the outcome; a quotable metafor call is what is missing and what would hold P6. |
| The first refit was wrong and that is the point | A plain REML/z fit reproduced the point, Q and I^2 exactly and returned 0.5356 to 1.1251 against the stored 0.3437 to 1.7532. Identical point and heterogeneity with a different interval means the inputs were right and the specification was wrong. The stored value was therefore investigated rather than overwritten, and it was correct: the interval is Knapp-Hartung on t(k-1) = t(2) = 4.3027 with the scale factor floored at max(1, Q/(k-1)), and with Q/(k-1) = 0.6288 the floor binds. |
| Prediction interval on t k minus 1 | This transcript is quoted verbatim and is not edited. metafor's predict takes its quantile from the FIT, and a default rma is test="z", so the pi.lb/pi.ub printed above are normal-quantile bounds. The Handbook interval for the same fit, on t_{k-1}, is given here beside it rather than in place of it. |
| Estimator note superseded | Paule-Mandel between-study variance with a Knapp-Hartung interval on k-1 degrees of freedom, floored at max(1, Q/(k-1)). The house rule refuses DerSimonian-Laird below k=10 and this pool is k=3. DL+Wald is carried alongside because it is what the prophylaxis half of this pair reported at k=4 -- so that half is now owed the same correction, and this note is the record of the debt rather than a silent divergence between two pages built the same night. |
| B two of the three are contributing a secondary outcome | On NCT03266783 the registered primary is clinically relevant bleeding and recurrent VTE is the third of nine secondaries. On NCT01780987 the registered primary is major bleeding and recurrent VTE is the first of five. Only CARAVAGGIO contributes its own registered primary. |
| C and the three comparators are three different drugs | Dalteparin, rivaroxaban, and unfractionated heparin followed by warfarin. The pooled quantity is therefore apixaban against whatever else was given, which is one step from any question a reader has. This object's GRADE rates indirectness down for it and the certainty here is very LOW. |
| Why the remainder is now zero and was 71 | It was 71 when the split was recorded and the whole remainder has since been screened -- 39 excluded and 16 sent to adjudication mechanically, the 16 adjudicated by hand. A remainder is not a queue to be drained; screening it is what turned 71 into a disposition table, and the shape of that table is the finding: 48 of 73 fail a criterion, and 39 of the 48 fail on population -- 30 at the mechanical screen and all 9 of the adjudicated exclusions -- because they are thromboprophylaxis trials in patients who have never had a venous thromboembolism. The surfacing query reaches across the treatment/prevention boundary because one search over one drug necessarily does; that is a fact about splitting a drug topic in two, not a criticism of the query, and the sibling review holds the trials this one excludes. |
| Why k included is 4 and k pooled is 3 | The object holds RAMBLE (NCT02829957), which the parent included and which cannot contribute -- n=19, both arms typed ACTIVE_COMPARATOR, registered primary a pictorial menstrual blood-loss chart -- together with the three trials that pool. It is kept and named rather than dropped, because a trial that fails only at the pooling step is a fact about this review's evidence base and dropping it would erase that fact. Eleven trials are eligible and poolable and only three contribute: the other eight post no arm-level count of the shared estimand, and each is named on the page. |
| Authority it does not establish | Mecir C5/C7. This boundary was written on 2026-08-19, after both reviews' included sets existed. `predefined: false`, like everything else in these criteria. |
| Indicator chosen because it could only move one way | P32: the test was its eligibility text, which states what was enrolled and cannot be true for both answers. A trial's own words about who may enter it settle whether the population has had the event; the arm label does not. |
| What this boundary does not do | It does not make the two reviews clinically homogeneous. Extended anticoagulation after a completed course and treatment of the acute event are both treatment under this criterion and they are different clinical questions with different comparators, which is why the treatment review pools within the acute stratum and states the extended trials separately rather than averaging across them. |
| Why it does not settle it alone | primaryPurpose is a registrant's coding and not a fact about the trial. ADVANCE-2 and ADVANCE-3 are one programme coded on opposite sides of it. The coded field decides which pool a trial is counted in; the trial's own design decides whether it is included, and where the two disagree the design wins and the disagreement is recorded. |
| And this review s own included trial barely meets them | RAMBLE (NCT02829957) is n=19, randomises rivaroxaban against apixaban with both arms typed ACTIVE_COMPARATOR, and its registered primary is a pictorial menstrual blood-loss chart. It is in scope by population and by intervention and it will not pool on this estimand. Stated here rather than discovered at the pooling STEP, because a review whose only included trial cannot contribute to its own estimand is a review whose k=1 is not the number a reader should take away. |
| What eligible poolable not included means | A gap in our own evidence base, not a gap in the literature. Each of the eleven was surfaced by this review's own search, passed its own criteria, and was not in the object. The disposition exists to name that state rather than let it read as an exclusion. |
| And the boundary was not applied to all of them until now | The boundary criterion reached the sixteen trials sent to adjudication. The nine admitted by the mechanical screen were admitted on `primaryPurpose`, the coded field these criteria say does not settle the question, and were put to the boundary for the first time in this build. HI-PRO survived it against a stated prediction that it would not. |
| And one domain here is worse than the ceiling | selection of the reported result is some concerns for every result in this review and for two of the three it carries an extra reason that is ours: the result used is a secondary outcome that this review selected from among the trial's registered ranks. The selection was made by a stated rule -- the shared estimand, chosen before any result was read -- and it is still a selection, and it is named rather than left for a reader to notice. |
| What is not established | The concealment mechanism is not on the registry and no publication was read. |
| Why this is not an adjudicated dispute | A blind second assessor from a different model family disagreed with this domain on 89% of judgements across 22 topics. That would ordinarily be a dispute to adjudicate. It is not one: the tool's own text names this exact evidential situation -- a bare statement that a study was randomised -- and prescribes the answer. The second assessor is not being deferred ; the guidance is being applied. |
| What the registry gives and does not | A ClinicalTrials.gov design module records `allocation: Randomized`. It carries no sequence-generation method and no allocation-concealment mechanism. The original judgement's own note said so before scoring LOW. |
| How this maps to the tool s algorithm | The tool maps 'No information' on 1.1 and 1.2 with no baseline concern to a domain judgement of some concerns. This project writes no information on the domain when it could not be judged from the sources read, and caps the overall at some concerns. Same place, and it keeps 'we could not judge this' distinguishable from 'we judged it and had concerns'. |
| What does not change | No overall rating and no GRADE CERTAINTY moves. All 51 judgements were already capped at some concerns or HIGH by another no information domain. |
| This does not replace the stored judgement | This is the judgement the published algorithm proposes from the signalling responses named here. It does not replace the stored `judgement` beside it, and nothing reads it yet. The tool itself says its algorithms "provide proposed judgements, but users should verify these and change them if they feel this is appropriate" -- so this is an input to a person, not an output to a reader. |
| Two of three contribute a secondary | On NCT03266783 and NCT01780987 the registered primary is bleeding and recurrent VTE is a secondary. This review pools recurrent VTE. Only Caravaggio contributes its own registered primary. |
| The comparators are three different drugs | Dalteparin, rivaroxaban, and unfractionated heparin followed by warfarin. Recorded here and not scored as a domain: RoB 2 asks whether a trial's own result is biased, not whether three trials answer the same question. This object's GRADE already rates indirectness down for it, in those words, and scoring it again here would double-count one fact as two defects. |
| And it bounds a finding of ours | Class 94 records assessor 2 saying no information on the randomisation process/deviations from the intended intervention/missing outcome data `100 of 100`. Measured across all 23 topics now stored that is 239 of 243, and all four exceptions are the randomisation process=some concerns on `agyw-hiv-prep-review` and `cab-prep-hiv-review` -- two of the three topics asked under the earlier prompt build, the one whose reply ids are bare NCTs. Under that build it is 4 of 6; under the later build 0 of 75. The 100% was measured on the later build's topics. The two-defaults mechanism stands; its universality does not, and what varies with it is the prompt. |
| The rate is still conditional on the same allow list | Both numbers are conditional on the fact list in THE_ALLOW_LIST_THE_RATE_IS_CONDITIONAL_ON above. Neither is a rate of agreement between assessors in general. |
| And the split itself shares nothing | The two split reviews share NO trial with each other, by construction: the boundary criterion assigns every trial to exactly one of them. That is worth stating precisely because it is the answer to a different question from the one above, and reporting it in place of the parent overlap would have been a true sentence standing where a false impression was needed. |
| Refusal discharges P46 because | It names what was reached (a PubMed search with its yield and its shortfall), what was found insufficient (no synthesis identified, and a search that retrieved 50 of 439), and what would have to be obtained. Re-derive with: python scripts/lint_refusal_citation_resolves.py |
| Which risk of bias domains drove the rating | Where a result is rated worse than LOW overall, the domain carrying that judgement is: measurement of the outcome on 3 result(s), selection of the reported result on 2 result(s). The overall rating is the worst domain, so these are the domains that determine it. |

Apixaban for the treatment of venous thromboembolism: eight trials report an outcome named for recurrent VTE, and three of them count the same thing1
titleQuestion. In adults with acute or recent venous thromboembolism, what is the effect of apixaban compared with conventional anticoagulation, another direct oral anticoagulant, or placebo on recurrent venous thromboembolism and on bleeding?1
Methods. ClinicalTrials.gov API v2 and PubMed (ncbi e-utilities esearch) were searched; estimates were pooled under a random-effects model with the REML estimator; risk of bias was assessed with RoB 2 (Cochrane risk-of-bias tool for randomized trials); and certainty was rated with GRADE.2
Results. Recurrent symptomatic venous thromboembolism -- nonfatal deep vein thrombosis or nonfatal pulmonary embolism -- with NO death term: risk ratio 0.776 (0.344 to 1.75) across 3 trials; 1 of these pooled outcome(s) carries a stated qualification -- a referral or a recorded finding -- given in full in Results and not reproduced here; and no statistical heterogeneity was detected (I-squared 0%).3
questionsearch.databases, screening.eligibility, results.by_outcome, risk_of_bias.by_outcome, grade.by_outcomeresults.by_outcomeBackground. This review asks: In adults with acute or recent venous thromboembolism, what is the effect of apixaban compared with conventional anticoagulation, another direct oral anticoagulant, or placebo on recurrent venous thromboembolism and on bleeding? It pools 3 randomised trials, against another anticoagulant. The outcome pooled is Recurrent symptomatic venous thromboembolism -- nonfatal deep vein thrombosis or nonfatal pulmonary embolism -- with NO death term.1

Primary trials. No bibliographic search for primary trials was run. The 4 included trials were identified by reading named registrations on the dates recorded on each entry.4
search.databases[0]search.databases[1]search.databases[2]bookkeeping_2026_08_21.the_search_its_date_and_its_databasesPopulation: adults with acute or recent venous thromboembolism being treated for it. Criteria are derived post hoc and say so.1
Each criterion above is recorded with the source it was derived from in `screening.eligibility_provenance`; none is asserted here without one.2

Yes at every rank, and the answer does not settle poolability1

Intervals are 95% confidence intervals.2
Risk of bias was assessed with RoB 2 (Cochrane risk-of-bias tool for randomized trials) (22 August 2019 version, as reproduced in the Cochrane Handbook), following Higgins JPT, Savovic J, Page MJ, Elbers RG, Sterne JAC. Chapter 8: Assessing risk of bias in a randomized trial. In: Cochrane Handbook for Systematic Reviews of Interventions version 6.5.1. A result, not a study -- Handbook 8.2. Every judgement below names the outcome it is about, and for two of the three that outcome is a secondary. 3 result-level assessments were made. A domain that cannot be judged from the registration and the posted results is no information, never LOW. Low-by-default asserts a fact; high-by-default invents a defect.3
A ceiling applies to every risk-of-bias judgement in this review; it is stated with the assessment itself rather than summarised here.4
Certainty of evidence was rated with GRADE, following Cochrane Handbook for Systematic Reviews of Interventions version 6.5.1, Chapter 14. HIGH for a body of randomised evidence, then rated down. 1 pooled outcome(s) were rated. No domain is rated up.5
results.by_outcome.recurrent_vte.model, results.by_outcome.recurrent_vte.estimator_usedconfig.confidence_levelrisk_of_bias.tool, risk_of_bias.by_outcomerisk_of_bias.ceilinggrade.approach, grade.by_outcomeThe pooled risk ratio for recurrent symptomatic venous thromboembolism -- nonfatal deep vein thrombosis or nonfatal pulmonary embolism -- with NO death term was 0.776 (0.344 to 1.75, 95% interval) across 3 trials, against another anticoagulant.1
Noted on this pool. This object's GRADE rates indirectness down for it and the certainty here is very LOW.2
The trials' results were closely consistent with one another: I-squared, the share of the variation between them that is more than chance alone would produce, was 0%; the estimated variance of the true effects between trials (tau-squared) was 0, the heterogeneity test statistic Q was 1.26, on degrees of freedom 2. computed; tau-squared is 0 and at k=3 that is an estimate from almost no information, which is why the interval is Knapp-Hartung on 2 degrees of freedom rather than a normal one3
The grounds for pooling are recorded as: Three trials randomise apixaban itself against another anticoagulant for the acute event and post an arm-level count of recurrent symptomatic VTE with no death term. Their definitions were read and compared, not their names. Where a later pool on this page rests on the same grounds, they are not restated.4
The full model output for every pooled outcome -- the call, the estimator, the heterogeneity statistics and the back-transformed interval -- is stored verbatim on this review object at `results.by_outcome.<outcome>.r_output.verbatim`. It is not reproduced as an Extended data section in this article; an earlier version of this sentence said it was, and no such section exists.5

HIGH for a body of randomised evidence, then rated down.1
No domain is rated up.2
This certainty rating is pending, not rated. It rests on a risk-of-bias judgement that this review has not finalised: two assessors read every contributing result independently, they disagree, and no adjudication has been performed, so this review holds no final risk-of-bias judgement for these results. Cochrane rates certainty per outcome by aggregating the risk-of-bias judgements of the results contributing to it, so a certainty rating cannot be more final than the risk-of-bias assessment it reads. The rating this object currently records, and the steps behind it, are shown below as the work so far rather than as this review's answer. No level is published for this outcome -- not the recorded one, not a more cautious one, and not a midpoint -- because choosing one would be the judgement that has not been made.3
Where a result is rated worse than LOW overall, the domain carrying that judgement is: measurement of the outcome on 3 result(s), selection of the reported result on 2 result(s). The overall rating is the worst domain, so these are the domains that determine it.4

A LOW i-squared does not show the trials measure the same thing -- on this very page the pool with the shared estimand comes out at 93.5%. Coherence is handled under indirectness, where it belongs.; indirectness: MODERATE to LOW, down 1 level(s) -- The three comparators are not the same comparator: dalteparin, rivaroxaban, and unfractionated heparin followed by warfarin. The estimand is shared and the contrast is not, so the pooled quantity is 'apixaban against whatever else was given', which is one step from any question a reader has. Stated rather than absorbed into heterogeneity.; imprecision: LOW to VERY_LOW, down 1 level(s) -- k=3, 3,933 participants, 61 events in total, and the Knapp-Hartung interval runs 0.34 to 1.75 -- consistent with a two-thirds reduction and with a three-quarters increase. This is the honest width and it is why the review's answer is 'not established' rather than 'no difference'.; publication bias: not rated down -- k=3. A funnel plot or Egger test at k=3 has no power and would be theatre. Not assessed, and not counted as absent. |

A relative effect cannot be turned into the benefit or the risk facing one patient without them, so this review can support a judgement about what these trials found and not a judgement about what to do.1

Writing one now and calling it prespecified would invalidate every other refusal on this page, because a reader who caught it would be right to disbelieve all of them.1
Eligibility criteria were derived post hoc when `apixaban-vte` was split in two on 2026-08-19, after the parent object's included set existed. The derivation is recorded element by element in `screening.eligibility`, and the criterion that separates this review from its sibling is stated in full at `screening.boundary_criterion`.2
Authority: Mecir R107 permits post-hoc eligibility criteria provided they are declared as such. They are declared, here and on the page.3
No PROSPERO registration or protocol record is held on this object. That is not the same as knowing this review was not registered, and neither claim is made. It is recorded here as an absence so that a reader does not have to infer it from silence.4

| property | state | reason |
|---|---|---|
| P1_executed_search | HELD | 3 database queries recorded verbatim with dates and counts; PRISMA arithmetic reconciles and the 0-trial unscreened remainder is stated as a number rather than omitted. |
| P2_k_cascade | HELD | k at every stage: surfaced 82, located 80, experimental 54, comparator 19, included 4, unscreened remainder 0. |
| P3_inclusion_criteria | HELD | Criteria provenance block present with predefined=False on its face (state 'DERIVED_POST_HOC'). |
| P4_preconditions | HELD | All 8 recorded with verdict and cited authority: 1 FAIL, 3 NOT-ASSESSABLE. criteria_predefined is FAIL -- post hoc criteria, which R107 permits and C5/C7 does not satisfy. |
| P5_extraction_table | HELD | 14 cells: 11 READ with source path and verbatim text, 3 DERIVED with the method named. |
| P6_analysis_output | HELD | Quotable REML/metafor output is held at results.by_outcome.recurrent_vte.r_output.verbatim, with the call recorded at results.by_outcome.recurrent_vte.r_output.call. |
| P7_published_comparison | REFUSING | No denominator yet. Blocked on external registration-id resolution; citation-string matching explicitly refused rather than substituted. |
| P8_registration_identity | HELD | 4 of 4 trial(s) verified live against the registry. |
| P18_restatement_is_reproducible | REFUSING | No restatement block on this cascade, so there is nothing to re-derive. This is NOT_ASSESSABLE dressed as a refusal rather than a pass: a topic that has never been restated has not demonstrated that a restatement of it would be checkable. |
| P19_promotion_reaches_derived_blocks | HELD | inputs.trials, k_cascade.k_included_in_object and prisma_flow.included all say 4; no first-person field in published_comparison declares a k the pooled outcome(s) do not. |
| P20_cascade_reconciles | HELD | k2_role_located=80 == k3+k4+k5, and k0_surfaced=82 == k3+k4+k5+kNA. Both checked, and every failing limb would be reported rather than the first. |
| P21_ambiguous_question_split | HELD | Split from 'apixaban-vte' into this review and 1 sibling(s) apixaban-vte-prophylaxis, with the decision recorded at None. Nothing was dropped to resolve the ambiguity. |
| P22_sharing_recorded | HELD | 2 shared trial(s), each recorded with the topics holding it and why: NCT02829957 in apixaban-vte; NCT03045406 in doac-cancer-vte-review. The object also states that summing per-topic k double-counts. |
| P23_recall_measured | HELD | All 3 recorded quer(ies) state their recall against this review's own included set: 1/1, 1/1 for THIS review, 1/2 for the parent and NOT MEASURED. |

| reconciliation | 82 surfaced = 54 experimental + 19 comparator + 7 background + 2 not-assessable; 80 of the 82 had a role located. 73 place apixaban in the randomised contrast and are this review's candidate pool. 1 was in the parent object and 72 were screened, so k_screened = 73. 48 excluded + 25 eligible = 73. Of the 25 eligible, 12 have posted no results, 2 are eligible and not poolable, and 11 are eligible and poolable. REMAINDER ZERO. |
| why_the_remainder_is_now_ZERO_and_was_71 | It was 71 when the split was recorded and the whole remainder has since been screened -- 39 excluded and 16 sent to adjudication mechanically, the 16 adjudicated by hand. The surfacing query reaches across the treatment/prevention boundary because ONE SEARCH OVER ONE DRUG NECESSARILY DOES; that is a fact about splitting a drug topic in two, not a criticism of the query, and the sibling review holds the trials this one excludes. |
| why_k_included_is_4_and_k_pooled_is_3 | The object holds RAMBLE (NCT02829957), which the parent included and which cannot contribute -- n=19, both arms typed ACTIVE_COMPARATOR, registered primary a pictorial menstrual blood-loss chart -- together with the three trials that pool. It is kept and named rather than dropped, because a trial that fails only at the POOLING step is a fact about this review's evidence base and dropping it would erase that fact. ELEVEN trials are eligible and poolable and only THREE contribute: the other eight post no arm-level count of the shared estimand, and each is named on the page. |

does each trial report a recurrent-VTE efficacy outcome at ANY registered rank -- primary, secondary or other -- before concluding which trials can be combined?
YES AT EVERY RANK, AND THE ANSWER DOES NOT SETTLE POOLABILITY
IT BOUGHT THE REVIEW ITS ENTIRE POOL. All three contributing results were found by asking at every rank: two of the three are SECONDARY outcomes in trials whose registered PRIMARY is a bleeding endpoint, so a review reading primaries only would have reported that they contribute nothing. IT DID NOT BUY POOLABILITY: eight trials answered yes and five of the eight still do not pool; the coherence_screen records why: NCT00643201 and NCT00633893 have no recurrent-VTE measure without a death term, NCT04168203 and NCT03196349 are extended-anticoagulation trials rather than acute-treatment trials, and NCT02744092 posts a percentage for a drug-class contrast rather than an arm-level apixaban count. The prophylaxis half of this pair ran the identical discipline and its poolable set went 3 -> 8. HERE IT WENT 8 -> 3. THE METHOD HAS NO DIRECTION -- asking is not a way of finding more trials, it is a way of finding out.
—
| registration | trial | two-component | three-component |
|---|
| review | poolable set | direction |
|---|---|---|
| apixaban-vte-treatment | 8 -> 3 | DOWN -- the question DISSOLVED a pool |
| apixaban-vte-prophylaxis | 3 -> 8 | UP -- the question RECOVERED a pool |
THE SAME QUESTION, ASKED THE SAME WAY, ON THE SAME DRUG, ON THE SAME NIGHT, MOVED ONE REVIEW'S POOLABLE SET UP AND THE OTHER'S DOWN. Every earlier instance in this corpus recovered a pool, and a check that has only ever returned the convenient answer cannot be told apart from no check at all. This pair is the refutation, and it arose on its own rather than being sought: it cost the treatment review the two largest trials in its field. Asking the withholding question is not a way of finding more trials. It is a way of finding out.
NOT that either pool is correct -- both were judged by reading endpoint definitions, and that judgement stands or falls on its own. NOT that the direction is unpredictable in general; two instances are two instances. What it establishes is narrow and it is the thing that was missing: THE PROCEDURE HAS NO BUILT-IN DIRECTION, demonstrated rather than asserted.

Denominator: —. Not established, and the obstacle is that no candidate synthesis has been identified for this estimand at all. The PubMed search at search.databases[2] reported 439 record(s) and retrieved 50; 389 were not retrieved and none of the 50 retrieved has been screened for whether it synthesises recurrent VTE on apixaban. There is therefore nothing yet to form a denominator over -- not a synthesis whose included set is unresolved, but no identified synthesis.
Explicitly not done: CITATION-STRING MATCHING. It would produce a comparison-shaped answer tonight and it is not identity: two syntheses citing the same trial under different labels would read as different, and the same label on different trials as the same. That is the substitution class this project audits for.
